2. Telephone Procedures
To streamline the routing of incoming calls, our office utilizes an automated phone system Monday through Friday, from 8:00am to 12:00pm and 1:00pm to 4:00pm. This automated system also allows any caller the option of speaking directly to a receptionist. Our staff access and act upon their voice mail messages several times throughout the workday. If you should call, after office hours, or on the weekend, your call will be directed to the doctor on call, who will be paged and she will respond as soon as possible. Of course, calls regarding an emergency situation will be handled promptly at all times by our staff.
3. Appointment Procedures
Patients are seen by appointment or for an emergency. An appointment can be made Monday through Thursday 8:10 a.m. to 3:30 p.m. at our Middletown office, by calling (502)245-8988. When making your appointment, please describe your condition to our staff. Please plan to arrive a few minutes early for your scheduled appointment. The doctors will always do their best to not keep you waiting. Please be sure to provide us with the telephone number where you can be reached during the day, in the event it is necessary to change your appointment. If you are unable to keep your appointment, please try to give us 24 to 48 hours advance notice, or a service charge may apply. If you will be delayed, please call us as soon as possible. Should you request that your appointment be rescheduled, we will offer you the next available opening. At every visit we ask that you bring the medication bottles for all of the prescriptions you are taking. This is necessary to ensure we do not prescribe a medication that would interfere with those ordered by another physician.
4. New Patients
We welcome all new patients. It is our goal to make your first visit as convenient and comfortable as possible. On your appointment date, please arrive 15 minutes earlier, to complete paperwork, and remember to bring a picture ID, Insurance card, list of all medications and the following completed forms:.

5. Referrals
Please contact our office to confirm that we currently participate in your insurance plan. If your insurance plan requires a referral from your primary care physician (PCP), in order to see our specialty provider, it is your responsibility to request and obtain this referral from your PCP. Your PCP may fax this referral to our office directly. However, if we do not receive this referral prior to your visit, you will be responsible for payment, or you will need to reschedule your appointment. Please call our office 48 Hours in advance of your appointment, to verify that your referral has been received.
6. Insurance and Fees
Our professional fees are well within the range of usual and customary charges for our specialty and region. We request and expect full payment at the time of service. All patients, including those covered under insurance plans in which we participate, are required to pay their co-payments, and deductibles at the time of service.
We currently accept Medicare B, Anthem BCBS, United Healthcare, Cigna, Humana and will be expanding our carriers.
We will bill your charges directly to any plan in which we participate. You will be expected to pay us directly, for any co-payment, deductible or charges which your plan determines to be the patient's responsibility. If you are not sure whether our physicians participate with your insurance plan, please ask one of our staff members. They will be happy to answer your questions.
You are ultimately responsible for charges incurred through our office should your insurance company fail to pay or delay payment.
7. Prescriptions
Please remind your provider of any prescription refills that you may require at the time of your office visit, when your medical records are readily available.
